
Healthy Sheboygan County (HSC) is a community-based initiative seeking to make positive changes in the health status of Sheboygan County. Members and representation in HSC includes public health, local health care agencies, schools, businesses, and community representatives that work together to help the community live better, longer lives.
History
HSC was formed by the Sheboygan County Division of Public Health in 1993 with a restructuring in 2004 and 2011.
Mission
To improve the health of the people of Sheboygan County-- leading to a community with "Everyone Living Better, Longer."
The Mission is accomplished by following these objectives:
Increasing the community's awareness of public health issues specific to Sheboygan County.
Identifying existing resources through organizations, agencies, citizen groups, and health care providers addressing specific public health issues.
Forming meaningful, action oriented coalitions of existing resources to reduce fragmentation effort in addressing Sheboygan County quality of life issues.
Mobilizing our community's residents in taking responsibility for adopting healthier lifestyles.
Action Teams have been created based on an assessment of the community's needs and the input of county residents. Currently there are six larger Action Teams:

Positive Mental Health
Responsible Substance Use
Access to Safe and Affordable Housing
Access to Affordable, High-Quality Child Care
