The 2023 Community Health Needs Assessment included a phone survey, community input survey, key informant interviews, listening sessions, and secondary data collection. The community health needs assessment provides Sheboygan County with information on the health status of county residents. Information within these reports include, but are not limited to, behavioral and lifestyle habits, risk factors, and disease conditions.
