
Leadership Council Members
The Leadership Council includes members of the community including, but not limited to, leadership from local community organizations relevant to the mission of Healthy Sheboygan County as well as Healthy Sheboygan County Action Team leads.
Mission:
To improve the health of the people of Sheboygan County leading to a community with "Everyone Living Better, Longer"
Purpose:
The Leadership Council provides oversight and contributes to the decision making process of the Healthy Sheboygan County Coalition and its various Action Teams.
HSC Structure
The structure and Action Teams within HSC are developed through a data driven approach to address the identified gaps or needs within our community. Every five years a formal Community Health Needs Assessment (CHNA) is completed collaboratively with the following partners: Sheboygan County Health and Human Services - Division of Public Health; Aurora Medical Center - Sheboygan County; Lakeshore Community Health Care; HSHS St. Nicholas Hospital; Froedtert Health; the University of Wisconsin, Division of Extension; and United Way of Sheboygan County.
The data gathered from the CHNA helps to inform the leadership council on which focus areas are a priority and provides direction for Action Team Leads. The current Action Teams are structured as pictured below.
