The 2023 Community Health Needs Assessment (CHNA) included a phone survey, community input survey, key informant interviews, listening sessions, and secondary data collection. The CHNA provides Sheboygan County with information on the health of county residents. Information within these reports includes, but is not limited to, behavioral and lifestyle habits, risk factors, and disease conditions.
